Personal Healthcare Advocate(PHA)

I combine my years of nursing and social work experience using a multi-systems approach to assist individuals who struggle with a variety of complex issues. My ultimate goals are to educate, advocate, coordinate, and access care for those individuals who are inundated with information that can be either confusing or overwhelming.

I can be a resource in or out of the hospital, long-term care/assisted-living, or office-setting. I can be the eyes, ears, and mouthpiece for families who don’t understand medical terminology, are unable to be present with a loved one, or who wish to find a neutral liaison. I am an expert at gathering information, presenting it in a simplified, concise manner, identifying the issues, setting goals, problem-solving, planning, and executing. I know how to achieve successful results.

Ultimately, the client makes the decisions. It is my job to present clear information, educate about options, and create an atmosphere where truly “informed” decision-making can be possible.

I am affiliated with CDPHC located in downtown South Bend. My work as a Personal Health Advocate has proved to be a unique asset to many patients, clients, physicians, employers, and employees alike.

The biggest difference between working with Mary C. Monnin, RN, LCSW and other providers is  she doesn’t just get partially involved, give advice and send the client on his/her way. Mary will walk the walk, with her client every step of the way from start to finish.
